ELIGIBILITY

1. Applicants must be among the following types of organizations: hospital, healthcare, community-based, faith-based, non-profit or for-profit organizations/facilities that have access to new fathers or soon to be fathers. Applicants are not required to have 501(c)3 status, nor to identify with a sponsoring organization with 501(c)3 status.
2. Applicants must be able to distribute within six (6) months 250 Daddy Packs™ to new fathers or soon to be fathers within their community.
3. Applicants must be willing to complete a one (1) page evaluation form within thirty (30) days of distributing the Daddy Packs™ that outlines how they were distributed and how they were received.
4. This evaluation form will be sent to the successful awardees via email.
