SUBMISSION GUIDELINES

• Answer each of the following questions in brief, but clear detail. Please number your responses; uniqueness and creativity counts.

1. (40 points) Provide information about the mission of your organization, and the services that your organization provides to fathers and families.  Also, briefly describe the demographic group your program targets or will target (including any relevant information about age, race, sex, socioeconomic status, geographic area, marital status, educational level, etc.).
2. (40 points) Provide information on how you will use the Daddy Packs™ to promote and/or enhance your existing services for families.
3. (20 points) Describe the method that you will use to monitor the distribution of the Daddy Packs™ and their effectiveness along with a distribution plan.

• Proposal must not exceed a total of two, single-spaced pages with 1” margins (top, bottom, and sides).  National Fatherhood Initiative prefers electronic submission of proposals.  Proposals must be in 12-point font and, if submitted via mail, on single-sided 8 ½” by 11” inch paper.  (Please do not use a binder, folder, report cover, or any kind of binding on the proposal; use a staple or paper clip if necessary.)  If you submit a proposal longer than two pages, National Fatherhood Initiative will consider the proposal non-compliant.

• Please signify within the proposal how many Daddy Packs™ you would like in English and in Spanish.
• Deadline: Completed proposals MUST BE RECEIVED by National Fatherhood Initiative no later than 5 PM EDT on July 31, 2007.
